Parents must complete the required registration forms prior to the first day of school:

• Identification and Emergency Information Form
• Pre-Admission Health History Form
• Physician's Report Form
• Consent For Emergency Medical Treatment
• Rate and Schedule Contract
• Field Trip Permission Slip

In accordance with Idaho law, health and age requirements must be met for children to enter preschool. A complete health check-up and all required immunizations must have been received prior to admission. A report of the health examine, which include an immunization record, must be properly filled out and signed be a licensed physician.
